Abstract

Traditional 3-point reduction may not restore proper projection of the zygomatic prominence following a comminuted or severely displaced fracture of the zygoma. In such cases, reconstruction of zygomatic arch as a 4th reference point for proper
reduction of fracture increases the accuracy of the reconstruction of zygomatic complex. Although the extended approach required to repair the arch increases operation time, its use is justified by the improved results in selected patients with
severely
displaced or comminuted fracture of zygoma.
16 zygomatic complex and adjacent structures have been exposed through a coronal incision following acute trauma or for correction of posttraumatic zygomatic deformity in our clinic from July, 1993 to August, 1994 and have been reconstructed
through
accurate reduction of zygomatic arch.

The treatment of the facial skeleton is difficult because teeth are too few for the fixation. Following fracture of the infantile facial skeleton, growth defects may appear caused by damage to the growth centers, sear formation and impaired
function.
In this case, we perfomed the ORIF and primary closure on Le-Fort ¥±combined with facial deep laceration in 8 year old boy duffered from traffic accident. After 1 year we removed piate on maxilla. Then we revised the scar tissues with elliptical
excision, Z-plasty, W-plasty and got the favorable result.
Finally, we got good cosmetic result using scar revision without significant functional restricion, growth impairment in this case.
